Our Care Managers not only check on their safety, but are there to listen to their concerns, coordinate care and solve everyday problems to help them remain in their home.

 

You will receive a CareReport from us after every visit which includes: A Home Fall Evaluation, An Overall Home Assessment Report, Service Recommendations plus an Overall Care Assessment.

When needed, we can connect you with community services including: Emergency Response, In Home Caregivers, Meals on Wheels and many other services. We will then oversee those services as they are delivered to your loved one.  Best of all. our fees are the most affordable in town, up to 75% less than other private care management companies.

While other private Care Management Companies can charge up to $150 per visit.  Our rates are set up for everyone to afford.

2 Visits per month = $100.00 per month

3 Visits per month = $150.00 per month

4 Visits per month = $200.00 per month

5 Visits per month = $225.00 per month
