CarePatrol selects the homes and facilities that we recommend to our clients with the upmost care. We examine each home's state survey record and only recommend those homes and communities that have score the least number of citations in each area. We may recommend a home or facility that is normally outside preferred homes if that home has demonstrated to us that they have satisfactory fixed all citations to our discretion.

Whether the participant has paid a fee to the referral service.
CarePatrol does not charge a fee to homes and communities if we are soliciting new homes for expansion of our network. However, a home may be charged a small fee to cover the time of our Senior Care Consultants if they are soliciting us to be part of our referral network.

The manner in which the referral service selects a particular participant from this group for that person.
CarePatrol matches our homes and communities that are part of our network by the requests of our clients including: location, financial criteria, ability to care for the client and other client directed requests.

The nature of the relationship between the referral service and the group of participants to whom it could make the referral.
CarePatrol does not own or have any financial arrangements other than our referral contract with any home or community.
Disclosure No. 5
The nature of any restrictions that would exclude such an individual or entity from continuing as a participant.
CarePatrol will eliminate a home or community from our active referral list if they do not maintain their low citation history, have a substantiated abuse or neglect compliant, loses their license for any reason or for some violations requiring a civil complaint or fine.
