Our Care Transition Managers Help Families in Lakewood Ranch, Venice, and Sarasota Understand the Things They Can Do Today to Prepare for Tomorrow

When exploring senior care transition options, most families need help to understand the complexity of the task. What type of care is appropriate? What skills do caregivers need to have? How will we pay for this? When beginning the process, it's OK not to know where to start, provided you have a team of experts that can help guide you. We started our CarePatrol franchise to provide direct help to families just like yours and make a profound impact on your loved one's life and happiness. We have helped thousands of families navigate the complicated process of finding the perfect care transition match in Lakewood Ranch, Venice, and Sarasota, and we can help yours as well. Let us help to educate you about your options across the entire transition process.

Senior Care Transitions Do Not Have Plug-and-Play Solutions

Care placement solutions are as unique as the people they are designed to help. Far too often, we see organizations in Lakewood Ranch, Venice, and Sarasota applying a broad-brush approach with their clients resulting in placements that do not meet their needs, or worst yet, are unsafe. With so much to consider when finding the perfect care match, it is crucial to dedicate the time to understand a senior's emotional, physical, and spiritual needs completely. During our no-cost eldercare consultations, we dig deep to understand your loved one's goals, hopes, and needs. We learn about their favorite activities, the level of socialization they require, and of course, their medical and physical needs. We build a complete picture of our clients to find the perfect transition solution that will make them happy, comfortable, and safe the first time.

Our Team Has the Skills to Deliver Holistic Senior Care Transitions

Our Care Transition Managers, comprised of nurses and social workers, are among the highest and best trained in the country. We have dedicated ourselves to continuous improvement through ongoing education and training. We hold senior care certifications, including:

  • Certified Senior Advisor (CSA)
  • Certified Transition Care Fellow (CTCF)
  • Certified Dementia Practitioner (CDP)
  • Certified Alzheimer's Disease and Dementia Care Trainer(CADDCT)

We are routinely asked to speak at national senior care conferences across the United States and dedicate ourselves to be a positive force in our field to improve the level of care all seniors receive.

Since a transition of care is often at the intersection of a major life-changing event, we have partnered with other professionals in Lakewood Ranch, Venice, and Sarasota that provide senior-based services. We have CPAs to help you understand and manage your senior's finances and elder care attorneys to assist with the legal challenges associated with a senior transition.

CarePatrol of Greater Sarasota is the team with the means, skills, and compassion that can help your family every step of the way. We are your resource when you need to find the right match for your loved one and a shoulder to lean on when you need the support of a friend.

How We Help You Find the Best Community for Your Senior

How We Help Tabs - CarePatrol - panel1

It all begins with a simple phone call. Why is this call so important?

Choosing a safer senior housing option, unfortunately, cannot be made by appearance alone. What is most important is something that really can’t be seen. It is "Can this community really care for your senior?" That's why CarePatrol's advisors complete a "Care Discovery" and base our recommendations on care options that can specifically meet your individual needs and preferences.

Guided Tour of Communities

We believe in private & personalized tours. We literally walk you through the steps. You will never get a list from us and be told to “go visit them alone, or on your own.” After we’ve matched your individualized needs to safer care options in your area, we will accompany you on a tour of each one of them. Or, we will provide a virtual tour if that works better for you. You are never alone because we will be your personal care guide from start to finish.

Full Care Details & Transparent Pricing

We insist on transparency with pricing & services. We don’t like surprises either. How to pay for care is always a concern and a valid question of our clients. Another important question relates to what services are provided in a specific care option. We can help alleviate these concerns. We will inform you of private, Veterans, or other government programs that you may be eligible for.

Moving Day & Transition Support

We work with the community so you can focus on your loved one. Leave the administrative work to us. Change can be hard, especially for Seniors. We make sure everything is in order so on moving day, everybody’s attention can be on making the transition as smooth as possible. Ensuring your questions are answered and you continue to feel good about your decision is as important as getting your senior settled in to a new environment.

Continuing Care Assessments

Personalized support that is really personalized. How can you trust an advisor with quotas? A Senior Advisor with "quotas" is something that you will never find at CarePatrol. The only “quota” we demand is satisfied families. Your advisor will be there for you when you need them - even after you make your decision. Long after move-in day, we’ll still be there with regular check-ins to ensure your loved one’s needs are being met.

Assisted Living Communities in Lakewood Ranch, Venice, and Sarasota: Finding the Right Blend of Social Needs and Physical Care

Assisted living communities are not what they were 20 years ago. These community-based care communities are focused on providing meaningful and engaging opportunities for your seniors to continue to live life at their pace while getting all the physical and medical care they require. Community-based care is centered around social interactions that keep your loved one active and feeling like they are a part of a group of their peers. When recommending an assisted living community, we look closely at your senior's current social and spiritual life as well as their physical and medical needs. Our goal is to find the right match where they will be happy for years to come.

In-Home Care: Independent Living in Lakewood Ranch, Venice, and Sarasota Allows For Seniors to Get the Care They Need in Their Own Homes

A dedicated care facility is not for everyone. Many seniors find that the right place for them to receive care is where they have always been, in their own homes. CarePatrol of Greater Sarasota has partnered with 47 in-home care providers who perform many helpful activities right in your senior's home. These caregivers can help with daily tasks like laundry and cooking, assisting with dressing, bathing, and toileting, and medication reminders. But these services often go far beyond assistance. In-home caregivers form close bonds with those they look after and provide much-needed socialization and companionship for isolated and lonely seniors. The key to a successful pairing of client and caregiver is to find common interests, likes, and personalities. When considering a caregiver, we will take extra time to learn about these traits in your loved one so that we can make the perfect match.
