Person-Centered Approach to Moving into Assisted Living

The needs of the whole person are so important to a successful move. We often fail to recognize when considering a move for our loved one the important intangibles that make their house truly a home. This includes the meaning of the home, the personal belongings and the control one has over their life while living independently in their home.

It is very important when considering a move, to plan carefully with the older adult, family and support network involved in the relocation process. It is also very important to familiarize the loved one with their new community.

The new place the adult senior moves into should reflect their life course. Items from their home give them meaning, and the new environment should reflect some of those features of their life.

Before choosing a new living environment, emphasis needs to placed on the individuals needs and preferences. What is the healthcare needs, strengths, interests, hobbies, spiritual preferences, and how close to family would they like to live?

Person Centered care coordination will achieve better outcomes for the loved one, while achieving respect, dignity and maintaining the highest amount of independence.
